Step 2: Pay Fees

To pay any state required fees, contact (915) 242-8575, – Release of Medical Information. For more information click here.

• Show proof of identity (copy of government issued photo with name)
• Fax completed form to (915) 242-8574

Step 3: Submit Your Request

Your request will be processed and mailed to the addres on the authorization form, or you may call to arrange pick up.
